REGISTRATION FORM

REGISTER EARLY FOR PROGRAMS
To run a good program, it is critical for everyone to sign up early! There are times that
classes must be canceled because there are not enough registrants. This decision must be
made by the deadline of the program so that those who may have signed up for the class may be notified.
Please, register early and carefully note program deadlines.

MINIMUM ENROLLMENT REQUIRED
In an effort to keep program fees as low as possible, minimum enrollment numbers have been
established for all programs. Programs not meeting the minimum enrollment number by the
registration deadline will be canceled.

PAYMENT
Full payment is required at the time of registration. Payment accepted by cash, check or money
order to the Town of Alton.
|
|
REFUNDS
Because the decision to conduct a program is based on the enrollment at the first class, no
refunds are guaranteed after the first meeting. Refunds will be issued if participants cancel
before the activity begins minus a $5.00 processing fee. Please note it takes two weeks to
process a refund. The Department does not confirm program registration, however, if a program is
full or canceled you will be contacted.

PHOTO POLICY
On occasion, our staff may photograph or videotape participants in department programs/classes or
at special events/facilities. These photos are for our use only and may be used in publications,
brochures, video productions or on this web site.

REC-PAK PROGRAM
Sign up for three or more programs per person at the Alton Bay Community Center and get a 5%
discount off total cost.

SUBMITTING REGISTRATION FORMS
MAIL-IN REGISTRATION:
Complete the registration form and mail it along with a check or money order (made payable to the
Town of Alton) to: Alton Parks and Recreation, P.O. Box 659, Alton, NH 03809. For a return
receipt please include a self-addressed envelope.
WALK-IN REGISTRATION:
Complete the registration form and bring it to the Parks and Recreation Building. Office Hours
are 7:00 a.m. - 3:30 p.m., Monday - Friday. The Parks and Recreation Department is located on
Route 11 across from Levey Park. Check out the kiosk and see the different programs going on,
extra registration forms are available in the kiosk. There is a mail slot in the office door
for 24-hour registration drop off.
